PDE4D (phosphodiesterase 4D; HGNC:8783, NCBI Gene ID 5144) is a human locus mapped to chromosome band 5q11.2-q12.1 that encodes cAMP-specific 3-prime,5-prime-cyclic phosphodiesterase 4D isoforms PDE4D1–PDE4D9 (809 aa PDE4D5; EC 3.1.4.53). PDE4D acts in gating of hippocampal cAMP-PKA-CREB1-BDNF long-term potentiation, cardiac RyR2 microdomain cAMP & beta-arrestin desensitization, together with PDE4A (Prefrontal Paralog Gene), PDE4B (Immune & Striatal Paralog Gene), and PDE4C (Ciliary Paralog Gene). Encodes cAMP-specific phosphodiesterase 4D, the principal regulator of hippocampal cAMP-CREB1 memory consolidation and cardiac ryanodine receptor microdomains.
Heterozygous UCR1/2 mutations cause Acrodysostosis Type 2; 5q12 stroke locus; target of allosteric memory enhancer zatolmilast (BPN14770) in Fragile X & Alzheimer disease
Gene Structure & Expression
Genomic Organization & Cytogenetic Locus: 24-exon mega-gene spanning >1.5 megabases on chromosome 5q11.2-q12.1 (one of the largest genes in the human genome), utilizing at least nine alternative 5-prime exons and promoters to generate nine distinct isoforms (PDE4D1–PDE4D9) that tether to beta-arrestin (PDE4D5), cardiac ryanodine receptor RyR2 (PDE4D3), and AKAP18/mAKAP scaffolds.
Cellular & Tissue Expression Profile: Abundantly expressed in hippocampal CA1/dentate gyrus neurons, cerebral cortex, brainstem area postrema, cardiac myocytes (within the sarcoplasmic reticulum RyR2 macromolecular complex), airway smooth muscle, and vascular endothelium.
Molecular Function & Pathways
Biochemical & Physiological Mechanism: Performs three critical microdomain functions: (1) in hippocampal neurons, long isoforms PDE4D4 and PDE4D5 restrict cAMP-PKA phosphorylation of CREB1 (Ser133) and BDNF transcription, acting as a molecular threshold for converting short-term memory into protein-synthesis-dependent long-term memory; (2) upon beta-2 adrenergic receptor (ADRB2) stimulation, beta-arrestin recruits PDE4D5 to the plasma membrane to degrade local cAMP and switch receptor coupling; and (3) in cardiomyocytes, PDE4D3 is anchored directly to the ryanodine receptor 2 (RYR2) complex to prevent PKA hyperphosphorylation and diastolic Ca2+ leak; during aging and heart failure, oxidative stress dissociates PDE4D3 from RyR2.
Clinical & Therapeutic Significance
Human Genetic Variants & Disease Phenotypes: Heterozygous de novo missense mutations in the UCR1/UCR2 autoinhibitory interface or catalytic domain of PDE4D cause autosomal dominant Acrodysostosis Type 2 (ACRDYS2), presenting with severe brachydactyly, nasal hypoplasia, and intellectual disability (without the PTH/TSH hormone resistance of PRKAR1A-driven ACRDYS1); common variants at 5q12 (PDE4D / STRK1) were also identified in Icelandic cohorts in cardiogenic and carotid ischemic stroke.
Therapeutic & Clinical Pharmacology: Whereas orthosteric pan-PDE4 inhibitors cause emesis by fully inhibiting short PDE4D isoforms in the area postrema, selective allosteric negative modulators of dimeric UCR2-capped long PDE4D isoforms—led by zatolmilast (BPN14770)—cap maximal inhibition at ~80% to avoid emesis while restoring hippocampal cAMP-CREB1-BDNF signaling, demonstrating significant cognitive and language gains in Phase 2/3 trials of Fragile X syndrome (FMR1) and early Alzheimer disease.
