CCK is produced by intestinal I-cells in response to fat and protein in the duodenum. The biologically active core is the C-terminal sulfated octapeptide CCK-8(s) — the clinical diagnostic agent is sincalide (synthetic CCK-8s). Activates CCK1R (Gq) on gallbladder smooth muscle → contraction + bile release, pancreatic acinar cells → enzyme secretion, and pyloric sphincter contraction / slowed gastric emptying. CNS CCK2R mediates satiety and anxiety-like effects. Very short plasma t½ (~3 min) — infused slowly to avoid cramping.
