Pharmacological Mechanism

Nucleotide reverse transcriptase inhibitor prodrug — converted to tenofovir → di-phosphate, a chain-terminator for HIV/HBV reverse transcriptase. Backbone of HIV ART + HBV antiviral. Renal proximal tubular toxicity + bone density loss are class concerns; TAF formulation (tenofovir alafenamide) reduces both.
