Pharmacological Mechanism

ACE inhibitor prodrug — hepatic carboxylesterase converts to ramiprilat. HOPE trial (2000) demonstrated CV-event reduction in high-risk patients independent of BP lowering — drove modern preferred use of ACEi for cardioprotection.
