Thionamide antithyroid — same TPO inhibition as methimazole + additional inhibition of peripheral T4→T3 conversion. Preferred over methimazole in first-trimester pregnancy (lower teratogen risk) and in thyroid storm. Hepatotoxicity warning at high doses.
