Irreversible non-selective MAO inhibitor — increases monoamines (5-HT, NE, DA) by blocking degradation. Tyramine-restricted diet mandatory (hypertensive crisis from cheese, aged meats, fermented). Reserved for refractory atypical depression + social anxiety. PK (Nardil label, single 30 mg PO dose): Cmax 19.8 ng/mL at Tmax 43 min, mean t½ 11.6 h, extensive metabolism via MAO (73% of dose recovered in urine as phenylacetic + parahydroxyphenylacetic acid within 96 h). Multiple-dose PK not studied in humans. PD-PK divergence: PD effect (irreversible MAO-A/B inactivation) outlasts plasma by weeks since target re-synthesis dictates effect duration — same caveat that applies to rasagiline and selegiline. F and Vd not in the label; estimates are textbook (~0.5 F first-pass, ~200 L apparent Vd from large lipophilic-amine class).
