Pharmacological Mechanism

Nitrofuran antibiotic — bacteria reduce the nitro group to reactive radicals that damage DNA + ribosomes. Concentrates in urine; used almost exclusively for uncomplicated UTIs. Pulmonary fibrosis + hepatotoxicity at chronic suppressive use.
