Thionamide antithyroid — inhibits thyroid peroxidase (TPO) blocking iodination of thyroglobulin tyrosines → halts thyroid hormone synthesis. First-line for Graves disease + thyroid storm (after PTU in early pregnancy). Agranulocytosis is the feared adverse effect.
