Pharmacological Mechanism

Peripheral µ-opioid agonist — slows gut motility via enteric neurons; P-glycoprotein efflux at the BBB keeps CNS exposure minimal at therapeutic doses. Abused at megadoses for euphoria — cardiotoxicity (QT, torsades) emerged as the predominant overdose harm.
