Pharmacological Mechanism
ACE inhibitor — blocks angiotensin I → II conversion in vascular endothelium. Lowers blood pressure, reduces afterload in heart failure, slows diabetic nephropathy. Renally cleared unchanged; no CYP metabolism. Cough (~10%) from bradykinin accumulation.
