Pharmacological Mechanism

Macrocyclic lactone antiparasitic — opens parasite glutamate-gated chloride channels → flaccid paralysis. Used for strongyloidiasis, onchocerciasis, scabies, lice. P-glycoprotein efflux at the BBB protects mammals from CNS toxicity. CYP3A4 substrate.
