Pharmacological Mechanism
Active metabolite of isosorbide dinitrate — bypasses hepatic first-pass nitrate conversion. Releases NO in vascular smooth muscle → cGMP → vasodilation; preferentially venous, reducing preload. Used for chronic stable angina + heart failure. Tolerance develops with continuous exposure → daily nitrate-free interval recommended.
