Pulsatile administration drives physiologic LH/FSH release; continuous administration paradoxically suppresses via GnRH-R desensitisation (the principle behind GnRH-agonist therapy for prostate cancer, endometriosis, precocious puberty). Commonly used in TRT protocols at 100–200 mcg SC 2×/week to maintain testicular function.
