Pharmacological Mechanism

Class IC antiarrhythmic — strong Na+ channel blocker with slow dissociation. "Pill-in-pocket" approach for paroxysmal atrial fibrillation in structurally normal hearts. Pro-arrhythmic in ischemic heart disease (CAST trial 1989) — contraindicated post-MI.
