Pharmacological Mechanism

Selective mineralocorticoid receptor antagonist — fewer anti-androgenic / progestational side effects than spironolactone (no gynecomastia). Approved for HFrEF post-MI + resistant hypertension. Hyperkalemia risk; CYP3A4 substrate (avoid with strong inhibitors).
