Class III antiarrhythmic — selective IKr blockade prolongs cardiac repolarization. Approved for AF rhythm control. Mandatory 3-day inpatient initiation (TdP risk during loading); strict CrCl-based dosing + numerous CYP3A4 / OCT2 inhibitor contraindications.
