Pharmacological Mechanism

Ribose is not a nucleoside per se but the pentose sugar backbone of purines and pyrimidines. Rate-limiting substrate for the pentose phosphate pathway feeding PRPP → de-novo nucleotide synthesis. Proposed to accelerate ATP regeneration after ischaemia / intense exertion by bypassing slow de-novo ribose-5-phosphate production. Clinical evidence modest (small CFS / fibromyalgia RCTs).
