Pharmacological Mechanism
Methylated morphine prodrug — analgesic effect requires hepatic CYP2D6 demethylation to morphine, the active µ-opioid receptor agonist (~10% of dose). CYP2D6 ultrarapid metabolizers risk respiratory depression; poor metabolizers get little analgesia.
