Fourth-generation cephalosporin — extended gram-negative coverage including Pseudomonas while retaining gram-positive activity. Empiric therapy for febrile neutropenia + healthcare-associated infections. Neurotoxicity (altered mental status, myoclonus) at high doses or renal failure.
