Pharmacological Mechanism

Azalide macrolide — 50S ribosomal inhibitor; expanded gram-negative + atypical coverage vs erythromycin. Long tissue half-life supports 5-day dosing. Modest CYP3A4 inhibition (less than erythromycin/clarithromycin). QT prolongation signal at high doses.
