The β-ketocarboxylate partner of BHB in the ketone-body pair. AcAc + NADH ↔ BHB + NAD⁺ via BDH1. Minor spontaneous decarboxylation to acetone (breath odour in ketosis). Tissue uptake via MCT1/2. Reactivated to acetoacetyl-CoA by succinyl-CoA:3-ketoacid CoA transferase (SCOT, extrahepatic) — liver lacks SCOT so it doesn't use its own ketones.
